6. Inspection report issued by the cosmetic inspection institution recognized by the Ministry of Health

7 Product packaging (including product labels)

8 Product Specifications

The entrusted entity shall submit a power of attorney for declaration

The following information should be provided for the record of cosmetics label:

1. Proof of the original label and translation that have passed the inspection

2. Chinese label proof

3. Copy of dealer's business license

4. Sample bottle
