(10) A copy of the power of attorney of the entity declaring responsibility for administrative license in China and a copy of the business license of the entity declaring responsibility for administrative license in China that has been filed with the official seal;

(11) Letter of commitment that the raw materials used in cosmetics and their sources meet the prohibition requirements for high-risk substances in mad cow disease epidemic areas;

(12) Documents certifying the production and sale of the product in the producing country (region) or the country (region) of origin;

(13) Other information that may be helpful to the administrative license.

In addition, there is 1 commercially available sample which has not been opened and sealed by the authorized inspection agency.

2. To apply for the record of import of non-special use cosmetics, the following materials shall be submitted:

(1) Application form for administrative License to import non-special use cosmetics;

(2) The basis for naming the Chinese name of the product;

(3) Product formula;

(4) Requirements for product quality and safety control;

(5) the original package of the product (including the product label and the product description);If you plan to design packaging for the Chinese market, you need to submit product design packaging (including product labels and product instructions) at the same time.

(6) Inspection reports and relevant materials issued by the authorized inspection institutions recognized by the State Food and Drug Administration;

(7) relevant safety assessment materials for substances that may have safety risks in the product;

(8) A copy of the authorization letter of the entity declaring responsibility for administrative license in China and a copy of the business license of the entity declaring responsibility for administrative license in China that has been put on record and affixed with the official seal;

(9) A letter of commitment that the raw materials used in cosmetics and their sources meet the prohibition requirements for high-risk substances in mad cow disease epidemic areas;

(10) Documents certifying the production and sale of the product in the producing country (region) or the country (region) of origin;

(11) Other information that may be useful for filing.

3. To apply for the administrative license of new cosmetic raw materials, the following materials should be submitted:

(1) Application form for administrative license of new cosmetic raw materials;

(2) Development report

1) Background, process and relevant technical data of raw material research and development;

2) Source, physical and chemical properties, chemical structure, molecular formula, molecular weight of raw materials;

3) Purpose, basis, scope and limit of use of raw materials in cosmetics.

(3) A brief description and sketch of the production process;

(4) Requirements for quality and safety control of raw materials, including specifications, testing methods, substances with possible safety risks and their control;

(5) toxicological safety assessment data, including relevant safety assessment data of substances with possible safety risks in raw materials;

(6) In case of agent declaration, a copy of the authorization letter of the entity declaring responsibility for administrative license in China and a copy of the business license of the entity declaring responsibility for administrative license in China that has been put on record shall be submitted with the official seal affixed;

(7) Other information that may be helpful to the administrative license.

Those who apply for cosmetic administrative license above shall submit relevant materials in accordance with the Provisions on Acceptance of Application for Cosmetic Administrative License. The general requirements of application materials are as follows:

(1) To apply for the administrative license of cosmetics for special use for the first time, one original and four copies shall be submitted, and the copies shall be clear and consistent with the original;

(2) One original copy shall be submitted for filing, extension, alteration or reissuance of approval documents;

(3) In addition to the inspection report, notarial document, official certification documents and third-party certification documents, the original documents of the declaration should be stamped by the applicant with the official seal or seal on each page;

(4) Use A4 size paper to print, use clearly distinguished marks, arrange in the prescribed order, and bind into volumes;

(5) the use of Chinese legal units of measurement;

(6) The contents of the declaration should be complete and clear, and the same item should be filled in consistently;

(7) All foreign languages (except overseas addresses, websites, registered trademarks, patent names, SPF, PFA or PA, UVA, UVB, etc., which must be used in foreign languages) shall be translated into standard Chinese and the translation shall be attached to the corresponding foreign language materials;

(8) The product formula shall be submitted in text and electronic version;
